Find out how our SMS 2.0 system tools can help your district comply with return to school requirements issued by Washington State Department of Health (DOH) and Washington State Department of Labor & Industries (L&I).

General guidance states that districts should not allow students, staff, vendors, parents, guardians or guests on site if they are showing symptoms of COVID-19 and/or have been in close contact with someone who has a confirmed or suspected COVID-19 in the last 14 days. For further information please refer to K -12 Schools- Fall 2020-21 Guidance and Considerations for K-12 Schools: Readiness and Planning Tool.

Custom Forms

Skyward Custom Forms are created to collect Human Resources and employee-related data. Users can also import data from files, adjust security to control access, and easily view and print entered data. This tool can be used for Health Screening required for all staff and students at entry each day.

Your district can choose to have an employee submit an initial Health Screening form and expect the employee to submit a new form only if health conditions have changed. Alternatively, your district can allow multiple forms per employee and require a new entry be completed for every workday.

The data gathered through Custom Forms can be viewed and reported on through Data Mining, the Custom Form browse screen, and Employee Profile. You can choose which fields/values to include for analysis of employee health data.

Online Forms

The Online Forms module allows the creation of a customizable step-by-step procedure for employees to complete when signed on to Employee Access. This tool tracks completion and provides the ability to mass email notifications related to the process. Your district can include various elements within the Online Form, for example, you can include a step for the employee to complete a Custom Form.
